Many extended health benefit plans will cover either a portion or all the cost of your custom-made prescription orthotics. It is your responsibility to know the requirements for eligibility. Some plans have a list of accepted diagnoses and covered treatment options, and some have specific forms that must be filled out. Check with your employer or with the administrator of your benefits plan provider about the details of your coverage. A physician prescription is generally requested for coverage.

Insurance providers do not answer questions from a non-plan-member.
Most insured clients must pay for the cost of their custom orthotics upfront and attach any supporting documents upon sending their claim to their insurance provider. Supporting documents include a prescription from a licensed medical doctor/physician. Your bio-mechanical and gait assessment form will be provided by us.
